Obituary of Karen Jean Forman-Fugate

Karen Jean Forman-Fugate, 69, of Union, KY, passed away suddenly after a stroke at St. Elizabeth Healthcare, Edgewood, KY on February 08, 2024.  Born in Covington, KY on March 23, 1954, she was the daughter of Donald Forman and Sonia [Ramsey] Forman.

Those left behind to share Karen’s love, light and legacy are her sons Andrew Allen Vance II, Michael Aaron (Kristy) Hartke,and Anthony Wayne (Bianca) Hartke; siblings Barry (Rea) Forman, JoAnn (Steve) Andrew, Donna (Jerry) Gunderson, Clyde Arnold “Arnie” (Susan) Forman, Timothy (Lisa) Forman, and Christy (Grant) Forman-Stidham; grandchildren Blake (Hailee) Vance, Austen Allen Vance, Carmella Charlette Hartke, Brennlee Angela Hartke, Bryer Anthony Hartke; great-grandchild Maisee Mae Vance as well as many other close relatives and friends.

Preceding Karen in death are her parents, brother Lanny Forman as well as sisters Linda Laake and Sherry Forman-Ricks.

A visitation will be held Saturday April 13, 2024 at Florence Baptist Church,642 Mt Zion Rd, Florence, KY 41042 from 10:00 AM until 12:00 PM.  Directly following, a memorial service will be held (also at the church) at 12:00 PM.
